CANCELLATIONS
We understand sometimes you need to change or reschedule your appointment. To ensure the time of our professionals is respected we require that you provide us with a 24-hour notification for all appointments. For cancellations made more than 24 hours before the appointment, the purchased credit(s) will be replenished in the guest's account. A "no-show", a request for a same day change, and any appointment canceled without proper notification (late cancellation) will result in a charge of 100% of the scheduled treatment. In the event that a same day change can be accommodated, it does not change The Stretch Spa, LLC's policy and should not create an expectation with the customer that The Stretch Spa, LLC can make such an accommodation again. All sales are final. The Stretch Spa, LLC accepts Visa, MasterCard, Discover and gift certificates.

OUR PRIVACY POLICY
We value your privacy and are committed to maintaining your security and confidentiality in the use of any information you choose to share with us. We do not disclose identifiable information to any third party without your consent. Further, we do not sell, rent, or otherwise allow the unauthorized outside use of personal information such as names, addresses, phone numbers, or e- mail addresses in our database without your permission. Copies of this form and signature will be valid as if original if this document is digitally scanned. If any part of this Release is found to be invalid by the courts having jurisdiction, or becomes inoperative for any reason, such invalidity shall not affect the validity and enforceability of any other provision of this release.
Monthly Memberships
All Monthly Memberships are for a three-month minimum commitment period and will automatically renew on a monthly basis until terminated or canceled (see Cancellation provision below).
Terms and Conditions
This Stretch Spa Monthly Membership Agreement (“Agreement”) is made by and between the The Stretch Spa (“Stretch Spa”) location designated as Your “Home Location” 1 (“we”, “our”and “us”), as described and subject to the terms herein, and the member listed above (“You” and “Your”).
